Once you've submitted your data, you will be given a SUB 5 ID number to affix to your DVD and a shipping address. Upon arrival, your DVD will be screened by our merchandising team and you will be notified in 4 - 6 weeks. If selected, you will then receive the SUB 5 contract. There will be no further action taken without your written consent.
